About Cortex Initiative
Founded in 2017 by Annie Tamburello, Cortex Initiative began with a mission to provide innovative, compassionate therapy services supporting communication, cognition, and overall well-being for adults. Building on that foundation, we now proudly serve pediatric clients as well, and our vision of expertise and care has grown into a trusted resource for children, families, and service members throughout the community.

Cortex Initiative Therapy Services
Cortex Initiative provides speech-language and occupational therapy for school aged children through our pediatric division, CortexKids. Our speech therapy services help students build strong communication, language, and executive functioning skills, while occupational therapy supports handwriting, fine motor coordination, and self-regulation. With engaging, personalized sessions, we help children gain the skills and confidence they need to thrive at school and in everyday life.

Cortex Initiative is excited to offer SLP and OT services through health insurance at CBA for the 2025-2026 school year.

Therapy Services Process

1. Submit Interest Form
   • Families begin by completing an online interest form to indicate their request for services.
   • Our staff will follow up with next steps and provide guidance on required documentation.
2. Referral
   • If services will be billed to insurance, a referral from the child’s physician is required.
   • A referral is not required for private pay/cash-based services.
3. Documentation
   • Parents/guardians must complete all required intake forms, consent documents, and provide insurance information (if applicable).
4. Evaluation & Plan of Care
   • Each student will undergo a comprehensive evaluation by a licensed therapist.
   • Following the evaluation, the therapist will develop a plan of care including treatment goals, recommended frequency, and duration of sessions.
5. Treatment Sessions
   • Therapy sessions are typically 30 minutes in length.
   • Frequency and duration will be determined by evaluation results and medical necessity, in accordance with insurance guidelines or parent preference (for cash-pay services).
   • Appointments will be scheduled during the school day, in coordination with school staff, so that students do not need to attend therapy outside of school hours.
6. Home Program
   • Families will receive a weekly folder with individualized activities to support skill development at home.
   • Consistent participation in home activities is strongly encouraged to maximize progress.

Policies & Records

1. Medical Records
   • All records will be securely maintained in our HIPAA-compliant electronic medical records system, SimplePractice.
2. Payment & Billing
   • Services will be billed to the child’s insurance when applicable.
   • Families are responsible for all co-pays, co-insurance, and deductible amounts as determined by their insurance plan.
   • Private pay/cash-based services are available at the posted private rate.
   • All patient responsibility amounts (co-pays, co-insurance, deductibles, or cash-pay fees) are due on the date of service and will be automatically charged to the credit card on file.

Therapy Services & Fees

We offer both insurance-covered and out-of-pocket options to make services flexible for families. All cash payments are due at the time of service.

Evaluation: Out of Pocket: $270
Treatment Session – 30 minutes: Out of Pocket: $75

How It Works

• Insurance Billed Rate: Amount billed to your insurance company. Your actual payment depends on your plan’s co-pay, co-insurance, and deductible.
• Prompt Pay / Superbill: Pay at the time of service and receive a superbill to submit to your insurance for possible reimbursement.
• Cash Pay: Pay out-of-pocket with no insurance billing. Ideal for families who prefer a simpler payment process.
