We Need Your Help: CBA Privacy Practices Form

We Need Your Help!

Due to an issue with our enrollment system, the Privacy Practices Form, including the Media Release, was not included in this year’s enrollment materials.

We are asking all CBA families to log in to the CBA Parent Portal and complete the Privacy Practices Form for their student at their earliest convenience. The form includes your preferences regarding student photographs and videos, the school yearbook, family directory, and handbook reference.

How to Complete the Form:

1. Log in to your CBA Parent Portal.
2. Select “Continue Enrollment Process” (green button).
3. Scroll down to “Privacy Practices Form.”
4. Select “Complete Form” (green button).
5. Complete each section of the form:
   • Media Release
   • School Yearbook Release
   • Family Directory Release
   • Handbook Reference
6. Select “Submit” (blue button) to complete the process.
